This form is simply for your information about what the Employee
Statement of Sickness looks like. You can print off a copy here it to give to
your employer. Your employer may (just may) give you this form to fill
in.
You will need to fill it in as soon as possible and give it back to
your employer. Keep a copy for yourself. This form will help your
employer to decide if you are entitled to SSP and whether they should
pay it.

If you decide to get a copy of this form - advised - and give it to your
employer, here are a few things to help you....
- If you cannot get SSP form your employer they MUST give
you the form SSP1 which will then tell you
why you cannot get SSP from
them. This form will then help you to get Employment and Support
Allowance benefits.
- If you do not agree with your employer's decision not to give
you SSP, then you can phone the helpline at 0191 225 5221. They will
tell you if your employer is in the right or wring, and also advise
you on the next step you should take. Don't be afraid to call this
number. They are normally very helpful - but also quite busy of
course!
- If you need further advice or help when you are sick, there is a
good leaflet available from and Jobcentre Plus or most post offices.
The form is called DWP 1026.
